Job summary

SWCHC, Inc. in Bridgeport, CT is seeking a qualified clinician to provide mental health assessment and treatment for children and adolescents within our school-based health center, with potential services at main sites.

The role includes psychosocial assessments, care planning, therapy, referrals, coordination with school and medical staff, advocacy in 504/IEP meetings, and thorough EHR documentation. A CT license and graduate degree are required; LCSW/LPC/LMFT preferred.

Qualifications

• Graduate degree in Social Work, Counseling or Human Services.
• CT License required; LCSW, LPC, LMFT preferred.
• Experience with Electronic Health Records (EHR) and documentation.

Responsibilities

• Provides psychosocial assessment, treatment planning, therapy, and referrals for youth ages 5–18.
• Facilitates group sessions (e.g. CBITS/Bounce Back).
• Refers clients for psychiatric care, SWCHC services, and outside providers as needed.
• Consults with supervisor, psychiatric providers, and medical staff regarding client care.
• Participates in behavioral health rounds, supervision, and meetings.
• Advocates for clients in the school system (504/IEP) and other systems such as DCF.
• Maintains client records in the EHR per SWCHC policies.
• Performs other duties as assigned.
