Job summary

Generations Family Health Center in Putnam, CT seeks a Clinical Therapist to provide comprehensive behavioral health services under the Behavioral Health Department and School-Based Health Center. You will perform evaluations, counseling, and treatment planning for patients across the lifespan, coordinating with medical staff and school-based programs.

The role requires a Master's in social work or related field, with licensed credentials preferred.

Qualifications

• Master's degree in social work or related field required.
• Licensed credentials preferred (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA).
• Experience with individuals, couples and families.
• Bilingual candidates preferred based on patient population.
• Experience with Substance Use Treatment is preferred but not required.

Responsibilities

• Conduct individual, group, family and/or couples therapy sessions.
• Develops and coordinates clinical treatment programs.
• Coordinate with medical providers and other clinicians.
• Assess patient progress and modify treatment plans accordingly.
• Coordinate care with referring physicians or agencies.
• Document therapies according to established protocols.
• Attend meetings and participate in committees.
• Maintain patient confidentiality and records.
