Job summary

Qualifications

• Graduate degree in Social Work, Counseling or Human Services.
• CT licensure required; LCSW/LPC/LMFT preferred.
• Experience serving medically underserved and culturally diverse populations.
• Ability to utilize Electronic Health Record (EHR).

Responsibilities

• Provide psychosocial assessment, treatment planning, therapy, and referrals for ages 5–18 (or until high school completion).
• Facilitate group sessions (e.g. CBITS/Bounce Back).
• Refer clients for psychiatric care and external services as needed.
• Consult with supervisor, psychiatric providers, and medical staff regarding client care.
• Participate in behavioral health rounds, supervision, and meetings.
• Advocate for clients in the school system (504/IEP) and other systems such as DCF.
• Maintain client records in the EHR per SWCHC policies; document as requested.
• Perform other duties as assigned.
