Job summary

Southwest Community Health Center Inc in Bridgeport, CT is seeking a licensed mental health clinician to provide individual and family therapy for children and adolescents in the school-based health center. You will conduct psychosocial assessments, develop treatment plans, and coordinate care with schools and other providers.

You will facilitate group sessions (CBITS/Bounce Back), coordinate referrals for psychiatric care, document in the EHR, and advocate for clients within the school system.

Qualifications

• Graduate degree in Social Work, Counseling or Human Services is required.
• CT license is required; licensure as LCSW/LPC/LMFT preferred.
• Experience serving medically underserved and culturally diverse populations.
• Ability to use Electronic Health Record (EHR).

Responsibilities

• Provides psychosocial assessment, treatment planning, therapy, and appropriate referrals to youth ages 5 through 18 (or upon completion of high school).
• Facilitates group sessions (e.g. CBITS/Bounce Back) as assigned.
• Refers clients for psychiatric care, Southwest services, and outside providers/services as necessary.
• Consults with supervisor, psychiatric providers, department members, and medical staff regarding client care.
• Participates in behavioral health child rounds, individual supervision, and department/agency meetings.
• Provides advocacy for clients within the school system (e.g. 504 and IEP meetings as requested by the client’s guardian) and other systems such as DCF.
• Maintains client records in the EHR in accordance with SWCHC timeframes/policies and provides other documentation as requested.
• Performs other duties as assigned.

SPECIFIC DUTIES AND RESPONSIBILITES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).

Requirements

• Computer literate including Microsoft Office; ability to document in an EHR.
• Ability to work well with culturally diverse populations.
• CPR/BLS Certified

Education and/or Experience:

• Graduate degree in Social Work, Counseling or Human Services.
• CT License required. LCSW, LPC, LMFT preferred.
• Experience in providing services to medically underserved and culturally diverse populations.
• Ability to utilize Electronic Health Record (EHR)
