Job summary

Enhabit Home Health & Hospice is seeking a Nurse Scheduler/Coordinator to organize patient scheduling with field clinicians in Springfield, IL. You will coordinate care with staff, patients, physicians, and referral sources to ensure seamless continuity of care.

You will also manage the on-call notebook and hospitalization logs to improve communication among stakeholders. Required are an LPN license, CPR, at least one year of clinical healthcare experience, a valid IL driver's license and

Qualifications

• High School Diploma or equivalent.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• Valid state clinical license.
• Minimum 1 year clinical experience in healthcare.
• CPR certification.
• 1+ year in home health, hospice, or pediatric care within last 24 months.
• Valid IL driver's license and auto liability insurance.
• Basic tech skills including mobile devices.
• Preferred: Homecare Homebase (HCHB).
• Preferred bilingual English/Spanish.
• In-office attendance required to achieve Enhabit objectives.

Responsibilities

• Schedule patients to field clinicians.
