Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to arrange patient appointments with field clinicians in Springfield, IL. You will coordinate with patients, physicians, and caregivers to ensure continuity of care and optimal outcomes.

The role requires strong communication, a valid nursing license as applicable, and reliable transportation. In-office attendance is essential to support teamwork and effective supervision.

Qualifications

• High School Diploma or equivalent is required.
• LPN/LVN license required per state rules.
• Valid clinical license and CPR certification are required.
• Minimum 1 year of clinical healthcare experience; home health/hospice experience preferred.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, and caregivers to ensure seamless care.
• Maintain on-call notebook and hospitalization logs to support effective communication.
