Fortis Care is seeking a Palliative Care Advanced Practice Provider (NP or PA) to serve as the primary clinician for residents in a Savannah SNF. Qualifikationen

• Licensed NP or PA in Georgia.
• Certification in Hospice and Palliative Care preferred (or willing to obtain within 12 months).
• Unrestricted DEA license preferred.

Aufgaben

• Provide palliative care consultations and follow-up with symptom management and plan of care.
• Lead goals-of-care and advance care planning discussions with patients and families.
• Collaborate with SNF care team to deliver timely, goals-aligned palliative care.
• Evaluate changes in condition promptly to support early intervention.
• Develop, update and communicate individualized care plans with interdisciplinary team.
• Support care transitions including post-hospital assessments and education.
• Maintain compliant documentation of assessments, care plans, and follow-up.
• Participate in quality initiatives to reduce acute-care utilization.
• Serve as a resource on best practices and symptom management.
