Job summary

Theoria Medical is seeking a part-time Advanced Practice Provider to serve skilled nursing facilities across the country, including Georgia. You will deliver high-quality patient-centered care, perform daily rounds, and coordinate with facility staff using an intuitive EHR and digital tools.

Qualifications

• Active U.S. license and board certification (ANCC/AANPCB/PNCB/AACN for NPs or NCCPA for PAs).

• Experience with skilled nursing facilities is a plus.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications and treatment plans.
• Communicate with facility staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and regulatory requirements.
