Job summary

Fortis Care seeks an Advanced Practice Provider (NP or PA) to lead on-site medical care at a Northeast Atlanta subacute rehab facility. The role features a fixed 8:00 a.m.–6:00 p.m. shift, four days per week, with no nights or weekend calls.

Responsibilities

• Provide direct medical care to short-stay post-acute rehab patients including assessment, diagnosis, treatment, and follow-up.
• Serve as the visible clinical presence during the 8:00 a.m.–6:00 p.m. shift on weekdays.
• Round proactively for new admissions, set expectations, and check in with patients and families.
• Manage on-site acute and chronic conditions to avoid unnecessary hospital transfers.
• Coordinate with leadership, therapy teams, specialists, and families to develop care plans.
• Support transitions of care, including medication reconciliation and discharge education.
• Collaborate with nursing, therapy, social work, and hospitality staff to ensure a high-touch experience.
• Communicate with surgeons, hospitalists, and PCPs to coordinate patient care.
• Maintain thorough and compliant documentation of patient encounters and plans.
• Participate in quality initiatives and patient satisfaction efforts.
