Job summary

Fortis-Care is seeking a part-time Weekend Nurse Practitioner to provide on-site clinical coverage at a skilled nursing facility in Georgia. The NP will focus on evaluating and managing acute weekend issues, delivering timely in-facility care and avoiding unnecessary ED visits.

The role is independent with physician support as needed, offering a consistent weekend schedule without overnight calls. Strong geriatric and acute care skills are essential.

Qualifications

• Current Georgia NP license and board certification in Family NP or AGNP.
• Unrestricted DEA registration.
• Minimum 1–2 years NP experience preferred, elderly care experience helpful.
• Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, urgent care, or emergency medicine is preferred.
• Familiarity with EHR systems and clinical documentation best practices.

Responsibilities

• Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.
• Respond to nursing staff-identified clinical issues and determine evaluation, treatment, and follow-up plans.
• Manage common acute conditions in SNF settings and decide on in-facility vs escalation to ED/hospital.
• Review labs, imaging, medications, and recent history to guide treatment decisions.
• Communicate clearly with nursing staff, physicians, residents, and families about changes and plans.
• Provide follow-up for residents recently discharged from hospital or with significant changes.
• Maintain timely, accurate, and compliant documentation of all encounters.
• Coordinate with Fortis Care clinical team for handoffs and continuity of care.
