Job summary

Ennoble Care seeks an experienced Advanced Practice Provider (NP or PA) to deliver primary and palliative care to homebound patients in the Savannah, GA area on a PRN basis. You will work under physician supervision, applying medical techniques to a diverse patient population with chronic disease.

Requirements include board-certified NP/PA and Georgia license, with 1–2 years of primary care or geriatric experience.

Qualifications

• Board Certified NP or PA with Georgia licensure.
• Experience delivering primary and palliative care to homebound patients.
• Strong assessment, diagnosis and treatment planning skills.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in Savannah area.
• Perform physical exams and develop care plans.
• Communicate with staff and clinical operations across systems.
• Order diagnostics and prescribe medications as appropriate.
