Fortis Care is seeking a Palliative Care Advanced Practice Provider (NP or PA) to serve as the primary clinician for residents in a Savannah SNF. This part-time position offers flexibility in daily hours, with potential to grow and add an additional facility in the Metro Atlanta area for those seeking more hours.
Join a physician-led team that emphasizes comfort, quality of life, and patient-centered care through expert symptom management and careful care planning.
The Palliative Care Advanced Practice Provider (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—will serve as the primary palliative clinician for residents in a single skilled nursing facility in Savannah, Georgia. This role is ideal for a provider passionate about improving quality of life for medically complex, predominantly geriatric patients through expert symptom management, serious-illness communication, and coordinated, patient-centered care.
This is a part-time position with significant flexibility. You may work a few hours each day, or opt for fewer, longer days without any overnight call. For APPs seeking full-time work, there is an opportunity to add an additional facility in the Metro Atlanta area.
In this role, you will collaborate closely with the SNF care team—including attending physicians, nurses, social services, therapy teams, and families—to deliver timely, goals-aligned palliative care that enhances comfort, reduces unnecessary hospital transfers, and supports thoughtful care planning.
At Fortis Care, we believe every patient deserves attentive, high-quality care delivered where they are. Our physician-led team partners with nursing facilities to provide expert, consistent, and compassionate medical services right at the bedside.
We take a holistic approach to health, combining expert clinical care with chronic care coordination to ensure patients’ needs are met across the continuum of care. By focusing on quality, efficiency, and teamwork, we improve outcomes and reduce unnecessary hospitalizations while treating every patient with dignity and respect.
As part of Fortis Care, you'll join a collaborative, mission-driven team that values independence, ownership, and doing the right thing for patients and partners. We invest in our clinicians, celebrate hard work, and provide opportunities to grow alongside a company that is redefining post-acute care.
