Job summary

Ennoble Care is seeking an experienced Advanced Provider (NP or PA) to deliver primary and palliative care to homebound patients in the Savannah, Georgia area on a PRN basis. You will assess, diagnose, and manage care under physician supervision, focusing on chronic disease management and compassionate patient support.

Candidate must be board certified with GA NP/PA licensure and have 1–2 years of PCP experience with geriatrics; hospice experience is a plus.

Qualifications

• Board Certified, with a valid NP or PA certification in the State of Georgia.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.
• Ability to operate independently and communicate effectively across teams.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Savannah, GA.
• Perform physical examinations and update medical histories to inform care plans.
• Develop impressions, diagnoses, and treatment plans.
• Prescribe medications and medically necessary interventions as needed.
• Coordinate care with families and caregivers during visits.
• Order diagnostic testing as medically necessary.
