Job summary

Care Transformation Collaborative-RI is seeking a Program Manager to support Rhode Island's Rural Health Transformation Program. This role leads project planning, provider engagement, stakeholder coordination, reporting, and implementation support to improve healthcare access and strengthen primary care across rural communities.

The position is full-time, hybrid (2 to 4 days in office per month), and requires a Bachelor’s degree with five years of healthcare project or program management

Qualifications

• Bachelor's degree required.
• Minimum five years of healthcare project or program management experience.
• Strong project management and communication skills.

Responsibilities

• Lead implementation of assigned program initiatives.
• Develop project plans and implementation timelines.
• Coordinate provider participation and stakeholder engagement.
• Facilitate meetings, workgroups, and collaborative activities.
• Monitor program performance and reporting requirements.
• Support technical assistance and quality improvement efforts.
• Coordinate contractors, consultants, and external partners.
