Job summary

Main Street Health is seeking a Health Navigator to work inside partner clinics. You will build trusted relationships with patients and providers, handling patient-facing, administrative, and logistical tasks to let doctors focus on care.

You will enter data, contact pharmacies, verify insurance coverage, and help coordinate follow-up care while maintaining compassion and collaboration with the clinic team.

Qualifications

Responsibilities

• Develop strong relationships with patients to support their care.
• Enter data into the Main Street software platform for reporting and tracking.
• Call patients to schedule visits or remind them to pick up medications.
• Order and schedule procedures, tests, and screenings as needed.
• Identify and track gaps in patient care and progress.
• Maintain records of patient interactions in electronic health records.
• Educate patients on care options and insurance benefits.
• Help patients access various community resources.
• Coordinate follow-up care after hospital discharge.
• Collaborate with clinic staff with compassion and attention to detail.
