Job summary

Main Street Health is seeking a Health Navigator to support rural patients and partner clinics inside clinic environments. You will build trusted relationships, handle patient-facing administrative tasks, enter data, and coordinate with pharmacies and insurers to support value-based care.

You will work full-time, Monday–Friday, 8am–5pm, traveling to partner clinics as needed. Training will be provided to use Main Street software and EHR systems to support reporting and care coordination.

Qualifications

• GED or High School Diploma.
• Self-starter who can work independently.
• Strong service mentality and community focus.
• Comfort using data to inform decisions.

Responsibilities

• Develop strong relationships with patients to assist with their care.
• Meet with patients during check-in; enter data into Main Street software for reporting.
• Call patients to schedule visits or remind them to pick up medications.
• Order and schedule procedures, tests, and screenings.
• Identify and track progress on gaps in patient care.
• Maintain records of patient interactions and communicate with providers via EHR.
