Job summary

Main Street Health is seeking a Health Navigator to support rural clinics by building trusted patient relationships and handling administrative tasks. You will work inside partner clinics to help patients navigate care, schedule visits, and coordinate services with pharmacy and insurance entities.

You will use Main Street’s software to document encounters, track progress, and assist with follow-up care after hospital discharge.

Responsibilities

• Develop trusted relationships with patients and providers.
• Enter data into the Main Street software platform for reporting and tracking.
• Call patients to schedule visits or remind them to pick up medications.
• Coordinate procedures, tests, and screenings.
• Identify gaps in patient care and track progress.
• Maintain records of patient interactions and communicate with providers.
