As a Health Navigator, you will build trusted relationships with patients and providers and handle patient-facing, administrative, and logistical tasks to support doctors delivering high-quality care.

You will work inside partner clinics, meet with patients, enter data into our systems, coordinate scheduling, and help with insurance and resources to improve patient outcomes.

Qualifikationen

• Must be a self-starter and able to work independently.
• Excellent communication and relationship-building skills.
• Comfortable using data to inform decisions.
• Familiar with multiple software platforms and EHR.

Aufgaben

• Develop strong relationships with patients to support their care.
• Meet with patients during check-in and enter data into the Main Street software platform.
• Call patients to schedule visits or remind them to pick up medications.
• Order and schedule procedures, tests, and screenings.
• Identify and track gaps in patient care.
• Maintain records of patient interactions and communicate with providers via EHR.
• Educate patients on healthcare options and insurance benefits.
• Help patients access community resources.
• Coordinate follow-up care after hospital discharge.
• Demonstrate compassion, adaptability, attention to detail, and teamwork.
