Job summary

Main Street Health is seeking a Health Navigator to support patients and partner clinics within a value-based care model. You will build trusted relationships with patients, enter data in the clinic software, and coordinate medication refills, tests, and follow-up care.

You will work full-time in a clinic setting, assisting with administrative and patient-facing tasks while enabling physicians to deliver high-quality care. Training and tools will be provided to succeed in this role.

Responsibilities

• Develop strong relationships with patients to assist with their care.
• Enter data into clinic software for reporting and tracking.
• Call patients to schedule visits or remind them to pick up medications.
• Coordinate orders for procedures, tests, and screenings.
• Identify gaps in patient care and track progress.
