Job summary

Main Street Health is hiring a Health Navigator to support rural patient care inside partner clinics. You will build trusted relationships with patients and providers, handle administrative tasks, and enter data into our software platform so doctors can focus on care.

Responsibilities include check-in data entry, scheduling, insurance coordination, and helping patients access community resources. A GED or high school diploma, a valid driver's license, and a full-time Mon–Fri schedule are

Qualifications

• GED or High School Diploma required.
• Active unencumbered driver's license required.
• Full-time, Monday–Friday, 8am–5pm schedule.

Responsibilities

• Develop strong relationships with patients and providers.
• Meet with patients during check-in and enter data into Main Street software.
• Call patients to schedule visits or remind to pick up medication.
• Order and schedule procedures, tests and screenings.
• Identify and track gaps in patient care.
• Maintain a record of patient interactions in electronic health records.
• Educate patients on healthcare options and insurance benefits.
• Help patients access community resources.
• Coordinate follow-up care after discharge.
