Responsibilities

• Coordinate release of information to all requestors in accordance with hospital policy and HIPAA laws.
• Verify authorizations per policy and regulations.
• Uphold security and confidentiality regulations for record access and review.
• Process and fulfill medical record requests via mail, fax, upload, or email; provide billing statements as applicable.
• Document status of requests for better communication with Business Office.
• Email monthly Mortality and SCI/TBI reports to the State Dept. of Health.
• Assist in other areas as needed.
• Review and analyze medical records for accurate coding of diagnoses and procedures.
• Assign codes for nursing home charges and clinic services per standards.
• Stay updated on coding guidelines and regulations.
• Collaborate with healthcare providers to clarify discrepancies in documentation.
• Conduct audits to ensure coding compliance and identify improvements.
• Provide training and support for staff on coding procedures.
