Job summary

Focused Post Acute Care Partners seeks an Occupational Therapist to assess patients and develop OT treatment plans in collaboration with physicians. The role includes administering therapeutic exercises, ADL retraining, and cognitive techniques, while supervising the OT Assistant if applicable.

The OT will coordinate with other health care providers to implement the plan of care and ensure high-quality patient outcomes.

Qualifications

• Current state license/registration as an Occupational Therapist.
• Ability to develop and implement OT treatment plans.
• Strong interpersonal communication and teamwork abilities.

Responsibilities

• Evaluate patients and develop OT treatment programs with physician collaboration.
• Administer active and passive therapies and ADL retraining.
• Supervise OT Assistant if assigned and coordinate with care team.
• Plan and adjust treatment plans based on patient status and goals.
• Ensure ethical practice, infection control, and regulatory compliance.
• Communicate effectively with patients, families, and other clinicians.
• Participate in interdisciplinary care conferences and care planning.
