StoryPoint in Wisconsin is seeking a Wellness Director to direct care services and coordinate clinical resident care. You will lead a team, optimize performance, and ensure care plans meet resident needs within our 1440 care framework.

Responsibilities include revenue management, admissions evaluations, regulatory compliance, and cross-team communication with EDs, sales, and external partners. A flexible schedule is required.

Formación

• RN license registered in Wisconsin is required.
• CDP certification required prior to or within 90 days.
• Assisted Living/Memory Care experience preferred.
• Leadership experience preferred.
• Ability to manage revenue and financials.

Responsabilidades

• Lead, manage and be accountable for your team and their performance.
• Oversee wellness revenue, evaluations, care conferences and service plans.
• Coordinate Alzheimer’s and dementia programs.
• Lead to 1440 care standards and communicate vision clearly.
• Partner with community leaders and external partners on action items.
• Market wellness through the Executive Director and Sales Team.
• Perform admissions evaluations every 6 months or with changes in condition.
• Ensure state regulations and audits are met.
• Obtain written orders for care plan changes.
• Verify MAR/TAR with pharmacy for accuracy.
• Review incident reports and address staff issues as needed.
• Communicate emergency policies and keep staff updated.
• Work toward continual organizational improvement.
• Be flexible with scheduling including on call, weekends, and holidays.
