Job summary

StoryPoint is seeking a Wellness Director to oversee clinical resident care services, delivering the 1440 experience and optimizing performance across the community. You will lead the wellness team, coordinate care plans, and ensure compliance with state regulations while maintaining strong partnerships with families and external collaborators.

The role requires LPN or RN licensure in Michigan, with CDP due within 90 days, and prefers memory care/assisted living experience as well as proven

Qualifications

• LPN or RN licensed in the state; CDP required within 90 days.
• Assisted Living/Memory Care experience preferred.
• Strong leadership and revenue management capability.
• Ability to solve problems and manage complex issues.

Responsibilities

• Lead and manage wellness team and performance.
• Ensure right care at the right price with staff and resources.
• Coordinate 1440 care standards and program guidelines.
• Communicate vision and build partnerships with residents, families, and external partners.
• Conduct admissions/evaluations and set measurable resident goals.
• Maintain compliance with state regulations and audits.
