Job summary

Devoted Health is seeking a Virtual Primary Care Nurse Practitioner to deliver high-quality care via telehealth, working Pacific Time hours and serving Spanish- and English-speaking patients.

You will manage chronic conditions, preventive screenings, and acute visits while coordinating with a multidisciplinary team to arrange in-person lab work, imaging, and referrals as needed. This role emphasizes patient-centered care and healthcare innovation.

Qualifications

• Active APRN or PA license in good standing and ability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with telehealth.
• Active BLS certification at time of hire.
• Shift times may vary to align with Pacific Time business hours; weekends possible.

Responsibilities

• Provide virtual primary care visits via telehealth including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.
• Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans.
• Evaluate and treat acute concerns within the scope of virtual care.
• Order and interpret lab tests, imaging, and other diagnostics; coordinate follow-up care.
• Manage medication prescriptions, titrations, and refills; ensure adherence and safety.
• Provide health coaching, lifestyle modification guidance, and patient education.
• Identify patients needing referrals or home-based services; coordinate with specialists or community resources.
• Utilize and provide feedback on our electronic health platform; participate in clinical case reviews and team huddles.
