Qualifications

• Bachelor's Degree in Nursing or Social Work required.
• RN license preferred or LVN/LPN license acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medi-cal and Medicaid programs is a plus.

Responsibilities

• Communicate regularly with residents and their families about care plans and treatment protocols.
• Attend morning meetings to facilitate team communication.
• Complete documentation and charting in a timely manner.
• Perform utilization review to ensure appropriate, timely care.
• Coordinate admissions, discharges, and referrals both within the facility and externally.
