This position is responsible for managing the skilled unit in accordance with current applicable federal, state, and local standards, guidelines, and regulations.
Job Description
- Provide direct and individualized nursing care to assigned residents based on established nursing protocols. Observe and assess pain/discomfort, oxygen therapy, drainage, dressings, and incontinence.
- Conduct systematic assessments of residents’ physical, emotional, social, educational, and functional needs through observation, interviews, and examinations.
- Interpret assessment data to develop a care plan.
- Ensure accurate transcription and timely processing of medical orders, integrating the medical care plan into nursing care.
- Administer medications correctly according to policies and procedures.
- Document care accurately and promptly, consistent with the resident’s plan of care.
- Administer medications as per physician orders, document on MOR, and monitor for adverse reactions.
- Organize resident charts and records, ensure completion of necessary forms and monthly summaries.
- Prepare incident/accident reports, notify relevant parties, and document appropriately.
- Assist residents in learning self-care activities.
- Supervise nursing team members’ assignments and communicate special instructions.
- Monitor meal services to ensure proper diet and assistance.
- Organize resources needed for the unit.
- Recognize emergencies and initiate appropriate responses.
- Ensure call lights are answered promptly and residents’ needs are addressed.
- Assist with orientation of new staff.
- Coordinate scheduled activities on the unit using activity or nursing personnel.
Qualifications
- Education: Bachelor’s Degree in Nursing or 1-2 years related experience or equivalent.
- Experience: 1+ years in a skilled, sub-acute, or memory care setting.
- Certifications: RN license from an accredited school.
- Language Skills: Fluent in English to communicate effectively with residents and staff.
- Technical Skills: Basic computer proficiency.
