• LVN/LPN license or RN license required.
• CCM certification a plus.

Responsibilities

• Communicate with residents and families about plans of care and treatment.
• Attend morning meetings to coordinate care with the team.
• Answer residents' questions about care and illness progression.
• Monitor and adjust residents' statuses as needs change.
• Organize daily work by assessing new and current needs; prioritize tasks.
