Job summary

PACS in Boca Raton, FL is seeking a licensed nurse (RN or LVN/LPN) to coordinate care for residents, communicate plans of care to residents and families, and collaborate with the healthcare team to ensure safe, cost-effective treatment.

The role requires organizing daily work, documenting activities, participating in stand-up meetings, and handling admissions and discharges while maintaining compliance with Medicare, Medicaid, and facility policies.

Qualifications

• Bachelor’s Degree in Nursing or Social Work is required.
• RN license preferred; LVN/LPN acceptable.
• Two years clinical nursing experience preferred.

Responsibilities

• Communicate regularly with residents and families about Plans of Care, PT, OT and other treatment protocols.
• Attend morning meetings/stand up to facilitate communications with the team.
• Answer residents’ questions about care, treatment plans, illness progression and other issues.
• Monitor and adjust resident statuses based on changing needs and conditions.
• Organize and prioritize daily work by assessing new, current and discharging residents' needs.
• Complete documentation as required.
• Perform utilization review activities to provide resident appropriate, timely and cost effective care.
• Coordinate care with resident, care providers, facilities financial services, and third party payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Accurate charting and ability to complete necessary paperwork in a timely manner.
• Ability to communicate effectively with physicians, staff, residents and families.
