Job summary

Qualifications

• Bachelor’s degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable with license.
• Minimum two years clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Accurate charting and timely paperwork.
• Continuing education/licensing must be maintained.

Responsibilities

• Communicate with residents and families about Plans of Care, treatments, and progress.
• Attend morning meetings to facilitate team communications.
• Answer questions about care, illness progression, and plans.
• Monitor and adjust resident statuses as needs change.
• Organize and prioritize daily work based on needs.
• Complete required documentation and utilization reviews.
• Coordinate care with residents, providers, facilities, and payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Maintain professional interactions with residents, families, staff and partners.
• Ensure efficient time management in a high-volume environment.
