Job summary

AdventHealth in Denver is seeking a Registered Nurse Transplant Coordinator to collaborate with a multidisciplinary team in providing comprehensive transplant care. You will manage patient care plans, document thoroughly, and coordinate UNOS-related steps from pre-transplant to post-transplant follow-up.

The role requires a nursing license, 1+ year in acute/critical care, and strong communication skills. Familiarity with UNet/UNOS and MS Office is preferred. This is a PRN role based in Colorado.

Qualifications

• RN license required
• Associate's degree in nursing required
• Demonstrated computer skills and EMR proficiency
• Strong communication and teamwork abilities
• Knowledge of organ transplantation processes preferred
• Experience in acute or critical care nursing preferred

Responsibilities

• Collaborates with multidisciplinary team to develop and implement individualized transplant care plans.
• Maintains confidential patient charts and records, documenting status, interventions, and outcomes.
• Facilitates UNOS listing, organ placement, and recipient admission processes.
• Coordinates transplant-related diagnostics, consultations, and post-transplant follow-up.
• Educates patients and families on transplantation processes and adherence to care plans.
