Job summary

Qualifications

• Collaborate with a multidisciplinary team to develop individualized care plans.
• Maintain thorough and confidential patient charts and records.
• Coordinate UNOS listing, organ placement, and recipient admission.
• Arrange diagnostic and therapeutic services, plus consultations.
• Provide direct transplant patient care in inpatient and outpatient settings.
• Assess patients' compliance with medications and treatment plans.
• Conduct pre-transplant interviews and interpret eligibility criteria.
• Prepare and analyze pre-transplant clinical data with medical staff.
• Educate patients and families on transplantation and post-care.
• Adapt to changing technologies, policies, and workload with flexibility.

Responsibilities
