Job summary

Eden Capital Careers is seeking a Clinical Manager in California. This role oversees patient care services, coordinates intake and care plans, and ensures safe, effective delivery of nursing services in home health settings.

Responsibilities include leading QAPI initiatives, maintaining survey readiness, and supervising clinical staff. Travel to multiple locations may be required; CPR certification and RN licensure are required.

Qualifications

• RN experience with at least 5 years in home health.
• 2+ years of supervision in a clinical/home health setting.
• Experience managing RN services in home, community or clinic.
• Valid CPR certification.
• Licensed driver with reliable vehicle.

Responsibilities

• Oversee day-to-day clinical and administrative office operations.
• Ensure regulatory compliance with state and federal regulations.
• Provide leadership and backup support in supervisor's absence.
• Coordinate patient care with interdisciplinary team to develop care plans.
• Drive quality and assessment; monitor transfers and discharges.
• Manage OASIS and discharge data; oversee discharge forms.
• Lead QAPI initiatives and track incidents with corrective actions.
• Maintain survey readiness and analytics; manage HHCAHPS surveys.
• Lead Emergency Preparedness Planning and monitoring.
