Trilogy is seeking an experienced RN Team Leader to join our Evansville, IN location. You will provide direct clinical care and lead the patient care staff to ensure safe medications, treatments, documentation, and care planning.

This role emphasizes teamwork, compassion, and adherence to nursing standards. Responsibilities include applying the nursing process from admission through discharge, daily resident assessments, and escalating changes to the charge nurse and physicians.

Qualifications

• Must have and maintain an active state RN license.
• CPR certification required.

Responsabilités

• Lead a team of direct care providers to ensure proper medications, treatments, documentation, family teaching, care planning and patient care conferencing.
• Use the nursing process to deliver care and ensure continuity from admission to discharge.
• Visit residents daily to assess status and notify appropriate staff of changes.
• Identify emergencies and initiate lifesaving measures when needed.
• Handle administrative duties such as forms, reports and charting.
