Trilogy Health Services, LLC in Vincennes, IN is seeking an RN to serve as a Team Leader, delivering direct clinical care and coordinating care across units. This role emphasizes leadership of direct care staff, patient safety, and adherence to health campus policies.

Responsibilities include supervising medication administration, documenting care, and ensuring smooth transitions from admission to discharge, while maintaining family communication and quality standards throughout the patient

Formación

• Must hold a current, valid state RN license.
• Current CPR certification required.

Responsabilidades

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition_
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician_
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
