Job summary

Century Care Management Inc. seeks a compassionate Registered Nurse with experience in long-term care to assess residents, develop care plans, and lead care teams. You will monitor patient conditions, assist with daily activities, administer medications, and collaborate with families and physicians to ensure high-quality, resident-centered care. A strong foundation in nursing processes and EHR documentation is essential.

Qualifications

• Graduate of an accredited nursing program (ADN or BSN) and current RN license.
• Experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred.
• Knowledge of nursing processes, care planning, and clinical assessment.
• Ability to assess residents and develop individualized care plans.
• Proficiency with electronic health records and clinical documentation systems.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record vital signs (BP, temp, HR, etc.).
• Monitor patients’ conditions and identify progress or adverse effects.
• Assist residents with daily activities (eating, bathing, etc.).
• Administer medications and provide basic clinical care.
• Collect samples for lab testing as assigned.
• Provide emotional support and communicate with families.
• Lead and supervise LPNs, CNAs, and staff as needed.
