Job summary

University Retirement Community in Davis, California seeks a passionate Resident Care Coordinator (RN) to join our Skilled Nursing Facility. The role supports nursing staff, collaborates with physicians and families, and ensures high-quality resident care.

Responsibilities include nursing assessments, patient advocacy, education, admissions/discharges, and care-plan updates. The position offers a daytime schedule with on-call coverage and competitive hourly wages.

Qualifications

• RN license; required to practice as registered nurse.
• Minimum of 1 year of professional nursing experience in a skilled nursing setting.
• Understanding of state, federal and local long-term care regulations.
• Current CPR certification.

Responsibilities

• Perform nursing assessments and advocate for residents.
• Provide patient education to residents and families.
• Cover the floor as staffing requires and support the nursing team.
• Assist with admissions and discharges and participate in care conferences.
• Create and update individualized care plans as needed.
