University Retirement Community in Davis, CA is seeking a passionate Resident Care Coordinator/ RN to support our skilled nursing facility. You will perform nursing assessments, advocate for residents, educate patients and families, and cover floor as needed.

Qualifications

• RN license required.
• Minimum of 1 year of professional nursing experience in a skilled nursing setting.
• Ability to work independently and make sound judgments in resident care.
• Knowledge of state, federal and local long-term care regulations.
• Current CPR certification.

Responsibilities

• Perform nursing assessments and coordinate care with physicians and inter-disciplinary team.
• Advocate for residents and educate patients and families.
• Cover floor duties as staffing requires.
• Assist with admissions and discharges and participate in care conferences.
• Create and update care plans and document interventions.
