Job summary

University Retirement Community is seeking a passionate Resident Care Coordinator for our Davis, CA skilled nursing facility. You will support nursing staff, collaborate with the interdisciplinary team and families, and help with admissions, discharges, and care plans on a 11:00 AM - 7:30 PM schedule with on-call shifts.

Required is an RN license in California, at least 1 year of SNF nursing experience, and CPR certification.

Qualifications

• Current California RN license.
• Minimum of 1 year nursing experience in a skilled nursing setting.
• Ability to work independently and make sound judgments.
• Understanding of state, federal and local long-term care regulations.
• Current CPR certification.

Responsibilities

• Support nursing staff and collaborate with physicians and the interdisciplinary team.
• Complete nursing assessments and advocate for residents.
• Provide patient education and assist with admissions/discharges.
• Create and update care plans; participate in care conferences.
