Job summary

Haven Hospice is seeking a dedicated RN to perform on-call visits in patients' homes in Chiefland, FL. This FT role includes evenings, weekends, and after-hours coverage with team support and ongoing training.

Responsibilities include delivering hands-on care, managing symptoms, and documenting care precisely while collaborating with physicians and the hospice team to ensure comfort and dignity for patients and families.

Qualifications

• Graduate of an accredited nursing program with active RN licensure in Florida.
• One year nursing experience required; 3+ years preferred.
• Hospice or related end-of-life care experience preferred.

Responsibilities

• Provide hands-on hospice nursing care in patients' homes during on-call hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications and adjust care per hospice protocol or orders.
• Communicate changes in patient condition to physician and obtain orders as needed.
• Offer guidance to patient and family on condition and next steps.
• Complete timely documentation and coordinate with hospice team.

As a On-Call RN You will:

• Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
