Haven Hospice in Jacksonville, FL is seeking an On-Call Registered Nurse to join our compassionate team in a full-time field position. The role involves after-hours, 1:1 hospice care for patients and families, delivering relief, clarity and compassionate support.

You will document care, assess symptoms, administer medications per hospice protocols, and coordinate with the physician. This is a stable role with team support and growth opportunities.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with a reliable automobile.

Responsabilités

• Deliver hands-on care to hospice patients and offer supportive services to families during after-hours on-call.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance to the patient and family in understanding the current condition and next steps.
• Pronounce death when required, provide family support, and notify the hospice team.
• Complete timely documentation and communication.

Schedule: FT Monday -Friday 4pm to Midnight. This is a field position.

Our On-Call RNs play a crucial role in end-of-life care, stepping in during off-hours to bring relief, clarity, and compassion when it's needed most. As a On-Call RN You will:

• Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication

Qualifications
