Job summary

Haven Hospice in Gainesville, FL is seeking an On-Call Registered Nurse to provide 1:1 end-of-life care in the patient’s home or facility during after-hours. You will collaborate with the hospice team to manage symptoms, administer medications, and support families.

The ideal candidate has an RN license in Florida, CPR certification, and hospice experience. This is a full-time, onsite role with a focus on compassionate care and clinical excellence.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured automobile.
• Possess and maintain current CPR certification where required.

Responsibilities

• Provide hands-on 1:1 hospice patient care during after-hours and on-call shifts.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols.
• Communicate with the physician regarding condition changes and orders.
• Support patients and families in understanding current condition and next steps.
• Pronounce death when applicable and coordinate with family and hospice team.
• Complete timely documentation and communication.

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication
