Job summary

Haven Hospice in Chiefland, FL seeks an On-Call RN to provide compassionate, 1:1 hospice care in patients’ homes with Friday evening to midnight, plus weekend shifts. You will work independently with strong team support, delivering symptom relief and family guidance while maintaining thorough documentation.

This full-time role offers flexible scheduling, benefits, and advancement opportunities within Haven Hospice’s growing home-care team in Florida.

Qualifications

• Graduate of an accredited nursing program with FL license.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice or home health experience preferred.
• Valid driver’s license and insured vehicle.

Responsibilities

• Deliver hands-on hospice care and support families after hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications and adjust care per hospice protocols.
• Communicate with physician regarding changes and obtain orders.
• Pronounce death if occurs during on-call and notify the team.
• Complete timely documentation and communication.

Office Location: Chiefland Coverage area: Chiefland and Surrounding Counties.

Schedule: FT Friday evening 4p to Midnight, Saturday and Sunday 8am-Midnight.

This position is going into patients homes and doing RN visits.

As a On-Call RN You will:

• Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
