Haven Hospice in Chiefland, FL is seeking an On-Call RN to join our hospice team. This FT role requires providing 1:1 end-of-life care in patients' homes, with evening and weekend on-call coverage as scheduled.

You will assess symptoms, administer medications per protocol, coordinate with physicians, support families, and document promptly. We offer comprehensive benefits and opportunities for growth within Haven Hospice, part of Adoration Health, with a strong team-focused culture and emphasis

Competenze

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

• Deliver hands‑on care to hospice patients and offer supportive services to their families during after‑hours, on an on‑call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on‑call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Office Location: Chiefland Coverage area: Chiefland and Surrounding Counties.

Schedule: FT Friday evening 4p to Midnight, Saturday and Sunday 8am-Midnight.

This position is going into patients homes and doing RN visits.
