Job summary

Haven Hospice in Jacksonville, FL is seeking an on-call RN to provide compassionate end-of-life care on a PRN basis. The role covers weeknights from 5pm to 8am and weekends across all shifts, with collaborative support from a hospice team.

You will deliver hands-on care, assess symptoms, administer medications, and maintain clear physician communication. The ideal candidate has an accredited nursing degree, FL RN licensure, CPR cert, and 1 year of nursing experience (3+ years preferred), plus a

Qualifications

• Graduate of an accredited nursing school with FL licensure to practice.
• One year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Must have a valid driver's license and insured vehicle.
• Maintain CPR certification where required.

Responsibilities

• Provide on-call 1:1 hospice care during after-hours and weekends.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols.
• Communicate with the physician to obtain orders as needed.
• Guide patients and families in understanding current condition and next steps.
• Pronounce death during on-call hours if applicable and notify hospice team.
• Complete timely documentation and communication.

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
