Hospice of the Midwest in Woodbury, MN is seeking a Hospice Admissions RN to join our patient-centered team. You will visit patients in homes, hospitals, or facilities, assess eligibility, and coordinate services with an interdisciplinary team while ensuring compassionate care.

This role offers a Monday–Friday schedule, competitive salary in the $92k–$95k range, comprehensive benefits, and opportunities for growth as part of a supportive, mission-driven organization.

Qualifications

• Graduate of an accredited nursing program with RN licensure in MN.
• 1 year hospice nursing experience required; 3+ years in related settings acceptable in lieu.
• Valid driver’s license with insured vehicle in good working order.
• Current CPR certification where required.

Responsibilities

• Visit patients referred for hospice care in homes, hospitals, or facilities.
• Explain hospice services and discuss goals with patients and families.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine hospice eligibility.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with the interdisciplinary team and maintain timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
