Job summary

Hospice of the Midwest in Edina, MN is seeking a Registered Nurse Case Manager to join our caring team. You will assess patients’ physical, emotional, and spiritual needs and create personalized hospice care plans that reflect patients' goals.

Responsibilities include administering medications, managing pain and symptoms, educating families, and collaborating with an interdisciplinary team. A valid nursing license in Minnesota and CPR certification are required; prior hospice experience is

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile and insured.
• Possess and maintain CPR certification where required.

As a RN Case Manager You will:
