Job summary

Hospice of the Midwest is seeking an On-Call Registered Nurse to provide 1:1 end-of-life care in Woodbury, MN. You will work 12-hour shifts on an alternating weekend schedule, delivering hands-on care, symptom management, and family support with a strong emphasis on compassionate communication.

Qualifications include MN RN licensure, CPR certification, and a driver’s license. Previous hospice experience is preferred, and the role offers competitive benefits and opportunities for professional

Qualifications

• Graduate of an accredited nursing program with current MN licensure.
• Minimum 1 year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Provide 1:1 hands-on hospice care during after-hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or orders.
• Communicate changes with the physician and obtain orders when needed.
• Pronounce death and provide family support if death occurs during on-call hours.
• Complete timely documentation and communication.

Schedule: Every other Sat & Sun, 12 hour shift (variable start times)

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
