Job summary

Hospice of the Midwest in Woodbury, MN is seeking a compassionate Hospice Admissions RN to join our team. You’ll visit patients in homes and facilities, discuss goals of care, and coordinate initiation of hospice services with a supportive interdisciplinary team.

Applicants should have an accredited nursing degree and active RN licensure in Minnesota, with hospice experience preferred. A valid driver’s license and CPR certification are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
