Job summary

Hospice of the Midwest in Woodbury, MN is seeking an On-Call Registered Nurse to provide compassionate, hands-on hospice care during after-hours. This role supports families with timely symptom management, medication administration, and clear communication with physicians.

Qualifications include MN licensure and 1 year of nursing experience; hospice experience preferred. Role may involve collaboration with Remote Triage and requires documentation and drive.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands‑on care to hospice patients and offer supportive services to their families during after‑hours, on an on‑call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands‑on care to hospice patients and offer supportive services to their families during after‑hours, on an on‑call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on‑call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication
